Abstract

Previous research stated that 84% of critically ill pediatric patients in the Pediatric Intensive Care Unit (PICU) had electrolyte imbalances. Children with severe malnutrition have low body potassium reserves due to inadequate intake and loss of potassium through diarrhea or vomiting. This is made worse by children suffering from infectious diseases, such as meningitis. The aim of this case study is to provide nutritional care to pediatric patients with severe hypokalemia, meningitis, and severe malnutrition by restoring electrolyte balance, improving nutritional status, and reducing the risk of re-feeding syndrome. The case study was conducted in May 2025 on pediatric inpatients in the PICU Room at RSPAL DR. Ramelan Surabaya. Research cases were selected based on the criteria of inpatients with complications of critical illness, namely hypokalemia, meningitis and malnutrition. Data collection begins with carrying out nutritional screening and the stages of assessment, diagnosis, intervention, monitoring and evaluation (ADIME). The methods used were observation, reviewing medical record data, and food intake interviews using the 24-hour Food Recall method for 3 consecutive days. The research results show that nutritional care with the TETP diet which is given in stages, both in meeting needs, in the form of food and in the route of administration to children with hypokalemia, meningitis and malnutrition, can provide improvements in potassium levels and energy and protein intake gradually reaching a target of at least 60% on the third day of the study. Monitoring and evaluating physical and clinical conditions such as muscle function, extremity movements, blood pressure, pulse, body temperature and patient's respiratory rate are in normal status. Weight gain cannot be evaluated due to the limitations of the patient's physical condition at the time of measurement, so it is recommended that hospitals use the Pediatric Advanced Weight-Prediction in the Emergency Room (PAWPER) tool.
